Registering with the Practice When you attend the Practice to register you will be asked to complete a GP registration form, new patient questionnaire form, and NHS Entitlement form, these must be completed before you see the Doctor,Please present all completed forms at the Practice, where the receptionist will offer you a new patient appointment with the Nurse or HCA. Acceptance on to the list is as at the discretion of the Doctor. We will ask you for proof of identity (see below). (Medical Cards are not accepted as proof of identity).If you default from your new patient appointment and do not in form the reception staff you will be refused acceptance on to the Practice list.There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away, if this is the case, simply choose another GP in your local area or you may not be entitled to NHS treatment.
New Registration PolicyIn accordance with Greater Glasgow & Clyde recommendations, all new patients should present two forms of identification which proves residency and authenticates their identity. This is required to reduce and prevent fraudulent attempts to obtain or misuse NHS care. Overseas visitors may be asked for additional information to prove NHS entitlement.One form of identity must ideally be photographic such as:• Passport• Drivers License• Official ID card from Public Services body• Student matdculation card (current year)Other documents for prove of residency that is acceptable are:• Recent utility bill (within last 3 months)• Council Tax document• Television License• Pay slips (last two months)• Rent book/agreement (Public Body or Private Landlord)• Bank Statement (Name and address section only required)• Solicitors Letter- (Clearly showing name and address)Once Registration and Acceptance is complete:Your medical records will be transferred to the Practice which may take up to several weeks to arrive at the Practice, and you’ll be sent a new medical card from GG&C Health Board.We thank you for your co-operation in making the registration process run smoothly;
In order to register with the practice you should bring your NHS medical card, if you have one.
The receptionist will ask you to complete a registration form and will offer you a registration medical appointment with either the Practice Nurse or the HCA.
If you prefer a particular type of doctor e.g. male or female we will do our best to note this, however not all doctors provide all services and a specific doctor may not be immediately available.
You may print off a registration forms, fill them out and return them to the practice. Alternatively, you can email them along with copy of proof of address and copy of valid ID to firstname.lastname@example.org
